IS IT NORMAL TO HAVE CURVED PENILE?

Penile Curvature is an abnormal bend in the penis that occurs during erection or in other words if the penis is bent to the left or right or up or down so much as to make it difficult or even painful to enter your partner during sex then it is matter of concern. This abnormal curvature of penis can be either Peyronie’s disease or Congenital curvature( since birth).

WHAT ARE THE SYMPTOMS OF CURVED PENIS?

  • Narrowing of the penis at particular area
  • Hard lumps in the penis/ plaque
  • Shortening of the penis.
  • The thickened area may be painful when the penis is erect.
  • It can be difficult to get an erection / loss of erection
  • The pain and the bending of the penis may physically prevent sexual intercourse.

 

WHAT ARE THE TREATMENTS AVAILABLE FOR CURVED PENILE?

The treatment options depend on several factors such as duratin of symptom, degree of penile curvature, presence of waisting (hour-glass deformity), size of penile plaque (fibrous scar tissue) and presence of erectile dysfunction. In case of mild curvature I suggest not to go for surgery as this type of Peyronie’s disease gets treated on its own.

Surgery is the only treatment known to be effective if :

  • The curvature is >30%
  • Curvature is stable for almost 6-8 months
  • Curvature is associated with Erectile Dysfunction.

PENILE CURVATURE COULD BE CORRECTED WITH THE FOLLOWING SURGERY

  • Penile plication surgery

Advantage – Low risk of erectile dysfunction

Disadvantage – Penile shortening

  • Penile graft surgery

Advantage- Penile length preservation

Disadvantage- Higher risk of erectile dysfunction

  • Penile prosthesis + curvature correction if curvature is more than 40%.

Advantage- Treat underlying erectile dysfunction. Mild degree of curvature gets corrected with penile implant.

Disadvantage- Risks associated with penile prosthesis

 

IS THERE ANY NON-SURGICAL TREATMENT AVAILABLE FOR CURVED PENIS?

 

In case of mild curvature Dr.Gautam Banga suggests not to go for surgery as this type of Peyronie’s disease gets treated on its own. Surgery is the only treatment known to be effective if curvature is significant.

 

WHAT ARE THE RISK FACTORS INVOLVED IN PENILE CURVATURE SURGERY?

Penile curvature correction surgery is considered a safe procedure when performed by expert surgeon. Surgical complications depend on the complexities of Peyronie’s plaque size and the type of curvature correction surgery.

Some of the common postoperative complaints include:

  • Temporary decrease sensation to the glans of penis
  • Potential penile shortening
  • Injury to the urethra
  • Development of erectile dysfunction
  • Recurrence of penile curvature (due to abnormal underlying penile tissue)

 

WILL I HAVE NORMAL FEELINGS AFTER SURGERY?

DOES A PENIS LOOK AND FEEL LIKE NATURAL ERECTION AFTER SURGERY?

Normal spontaneous penile erection is common after penile curvature surgery but any sexual activity is discouraged for atleast 4-6 weeks.

 

WHAT ARE THE PRE AND POST- SURGERY PRECAUTIONS?

 

It is usually recommended not to get involved in sexual activities for 4-6 weeks post-surgery.

Strenuous exertions are to be avoided. Patient should be realistic about surgery results.

 

HOW MANY DAYS HOSPITALIZATION IS REQUIRED FOR THIS SURGERY?

Usually 2 days hospitalization is needed for a patient following Penile curvature surgery. But we allow such patients to get discharged from hospital once they are In stable condition. And they are recommended to come after a week for follow up.

 

WHAT IS THE RECOVERY TIME AFTER PROCEDURE?

Recovery is fast and the patient can resume routine work within 24-48 hours.

 

WHAT IS THE COST OF THE SURGERY?

Surgery cost depends upon certain factors like degree of curvature, severity of the ailment, penile implant.

Urethral Injury Treatment

What is urethral injury?

Urethral Injury is when the urethra is hurt by force. The male urethra is lot more prone towards injury than the female urethra. Symptoms of urethral injury include swelling in perineum area, pain with voiding or inability to void. Complications can also occur which is infection, erectile dysfunction, incontinence and stricture formation

What are the symptoms of urethral injury?

Symptoms can been felt and seen from outside. Some of them are as:-

  • Blood at the urethral meatus is the most important sign.
  • Poor urine stream.
  • Painful urination
  • Swelling in the penis and perineum region.
  • Lower abdominal pain
  • Inability to pass urine.
  • A feeling of incomplete emptying of bladder
  • Swelling of the testicles (in males only).
  • Pain while ejaculating (in males only).
  • In women, urethral syndrome can also cause discomfort in the vulvar area.

Urethral Injury

Will the injury or the surgery cause problems with sex?

Yes it will cause sexual problem. If the injury is proximal (close to bladder) it can lead to poor erection/ erectile dysfunction. If injury is distal to bladder then it can lead to poor ejaculation and reduced semen volume.
Once surgery is performed, then the patient can resume to normal sexual life within 2 months post-surgery provided surgery is done by an expert.

Will the injury / surgery create problem in urination?

Bladder Neck injury can lead to incontinence.Urethral injury can also lead to stricture formation.Acutely, it can lead to retention of urine with extravagation (perineum swelling). In case of straddle injury, direct impact to perineum with blood at meatus.

How urethral injury can be diagnosed?

A dye study X-Ray Retrograde Urethrogram (RGU) + Micturating Cystourethrogram (MCU) are often done after urethral injury.

What all are the treatment available to cure urethral injury?

The treatment of a urethral injury is very technically challenged. The injury can be successfully repaired with surgery in most cases. In

In case of acute injury the bladder may be drained by a catheter through the urethra or the abdominal wall over a period of days to weeks. This will prevent urine from building up in the bladder.

It will also allow the injured bladder or urethra to heal and prevent swelling in the urethra from blocking urine flow.
Suprapubic cysostomy is done if catheterization is not done. Primary realignment of urethra can be attempted within 2-3 days of injury. Urethral repair is planned 4 months post injury.

What are the precautions to be taken care before urethral surgery?

Check for urethral defect / displacement,  erectile profile of male patient,sensation over penis and glans.

What can a patient expect after surgery?

If injury is treated by urologist specialized in Genito-Urinary reconstructive urology, patient can resume his routine social as well as sexual life within 2 months after surgery.
