Penile Implant Surgery as done by Dr. Gautam  Banga

26 years old Angad was on cloud nine. He just landed on his dream job and was getting groomed to proceed to the United States in the next six months, for a complete two years deputation. His success and upcoming deputation meant only one thing for his family; it was the time that they started searching for a suitable partner for him.

Angad has always been a straight arrow; an A list student, topper of his college batch, MBA from a reputed national institution, and had no prior relationships. He always remained focused on his career goals.

A generally shy and reserve natured Angad vehemently tried to oppose his family’s proposition stating various reasons; reasons were like- ‘just landing on my dream job’, ‘I am not ready yet’, ‘would think about marriage post returning from US’, etc. Despite giving such reasons to his parents, all such reasons fell on deaf ears. Finally, he gathered all the courage he had and confided in his father the real reason he did not want to get married.

Angad told his father that he suffers from erectile dysfunction. He had been suffering from this condition since he hit puberty. All these years he had not disclosed this private information thinking about the mocks and laughs which will be directed to him. Angad had not been into relationships due to the fact of him thinking that he would not be able to perform well during the intimate moments. He always feared that any such encounters might just unmask his well-kept secret of so many years.

After knowing the truth, Angad’s father took him to their family doctor. Listening to the problem at hand, Angad’s family doctor first informed them that a corporeal cause can be recognized in about 80% of cases of erectile dysfunction. The doctor further clarified that mental impotence is where erection or penetration fails due to thoughts or feelings; which has accounted in about 10% of cases. He further suggested a Penile Implant as the solution. He guaranteed that this was the best solution available to their problem.

Seeing that Angad and his father were still skeptical about a surgery, their family doctor suggested Dr. Gautam Banga, who took over the case and explained that this is also known as penile prosthesis, were devices used to treat severe erectile dysfunction. Such surgery comes into the picture only when the case of erectile treatment is very extreme and all other approaches, including hormone injections, have been unsuccessful.

The doctor also cautioned them that such a surgery is usually kept as the last resort because it is a permanent and irreversible solution. He also informed them about the process of this surgery. The doctor further clarified in presence of Angad and his parents, regarding the process of penile implant, mainly basing upon their type. He substantiated the two types:- semi-rigid and inflatable.

The semi-rigid type comprises of only two bendable rods inserted in the chambers within the penis. This leaves the penis in a state of semi-erection always. There are zero risks of failure in such implants, and the process is very simple with minimal chance of not being successful. However, continuous semi-erection can become awkward & embarrassing for the patient. Furthermore, the patients are inept to achieve a full erection, which might disrupt their sex lives.

Inflatable penile implants are a more contemporary answer to the problem of erectile dysfunction. As an alternative of rods, hollow cylinders are inserted within the penis chambers. The cylinders are linked to a sack filled with fluid and a pump (usually located in the scrotum). The patient can use the pump as and when he wants an erection. The motion forces the fluid into the hollow cylinders, expanding them and giving the appearance of an erection. The same pump can be used again to transfer the fluid back into the reserve.

Angad and his father were also explained the risks & drawbacks and advantages of penile implant surgery. The reason why such implanting surgeries are getting widespread is due to the numerous advantages they offer to men. The doctor while explaining stated that, while the apparent benefit is getting rid of erectile dysfunction, many other benefits make such surgery an improved treatment compared to other types of treatments:

  • The implant can be inflated in secret and are easy to hide. Most patients report no visible change in their daily lives.
  • Intercourse is more pleasing than all other treatments of erectile dysfunction.
  • Due to limited parts, the mechanical failure rate is virtually insignificant
  • There is no risk of extended erection or priapism.
  • No sensation on the skin is lost, and men can reach orgasm the same as before.

Danger and Disadvantages of Penile implant

The doctor made Angad and his family aware of all the dangers so associated with this surgery. Angad was made to understand that once the surgery is completed, all-natural impulses and responses for erection in the penis are lost. It means that the man would not be able to attain an erection without the implant at all. Other things which he was explained by the doctor were-

  • It only deals with the erection size and time. They do nothing for the penile size and form, and any prevailing malformations in that area would have to be treated separately.
  • There are probabilities of undue bleeding just after the surgery.
  • The erection time might be shorter than normal with consistent  implants.

Post all these cautionary advice and consultation, Angad was operated by one of the best urologist in India, none other than Dr. Banga.
