What is urethral injury?

Urethral Injury is when the urethra is hurt by force. The male urethra is lot more prone towards injury than the female urethra. Symptoms of urethral injury include swelling in perineum area, pain with voiding or inability to void. Complications can also occur which is infection, erectile dysfunction, incontinence and stricture formation

What are the symptoms of urethral injury?

Symptoms can been felt and seen from outside. Some of them are as:-

  • Blood at the urethral meatus is the most important sign.
  • Poor urine stream.
  • Painful urination
  • Swelling in the penis and perineum region.
  • Lower abdominal pain
  • Inability to pass urine.
  • A feeling of incomplete emptying of bladder
  • Swelling of the testicles (in males only).
  • Pain while ejaculating (in males only).
  • In women, urethral syndrome can also cause discomfort in the vulvar area.

Urethral Injury

Will the injury or the surgery cause problems with sex?

Yes it will cause sexual problem. If the injury is proximal (close to bladder) it can lead to poor erection/ erectile dysfunction. If injury is distal to bladder then it can lead to poor ejaculation and reduced semen volume.
Once surgery is performed, then the patient can resume to normal sexual life within 2 months post-surgery provided surgery is done by an expert.

Will the injury / surgery create problem in urination?

Bladder Neck injury can lead to incontinence.Urethral injury can also lead to stricture formation.Acutely, it can lead to retention of urine with extravagation (perineum swelling). In case of straddle injury, direct impact to perineum with blood at meatus.

How urethral injury can be diagnosed?

A dye study X-Ray Retrograde Urethrogram (RGU) + Micturating Cystourethrogram (MCU) are often done after urethral injury.

What all are the treatment available to cure urethral injury?

The treatment of a urethral injury is very technically challenged. The injury can be successfully repaired with surgery in most cases. In

In case of acute injury the bladder may be drained by a catheter through the urethra or the abdominal wall over a period of days to weeks. This will prevent urine from building up in the bladder.

It will also allow the injured bladder or urethra to heal and prevent swelling in the urethra from blocking urine flow.
Suprapubic cysostomy is done if catheterization is not done. Primary realignment of urethra can be attempted within 2-3 days of injury. Urethral repair is planned 4 months post injury.

What are the precautions to be taken care before urethral surgery?

Check for urethral defect / displacement,  erectile profile of male patient,sensation over penis and glans.

What can a patient expect after surgery?

If injury is treated by urologist specialized in Genito-Urinary reconstructive urology, patient can resume his routine social as well as sexual life within 2 months after surgery.
